PharmacologyMethadone is a long-acting, inexpensive, synthetic opioid developed in the 1920s that has many uses in the palliative care setting as it treats multiple types of pain including neuropathic and somatic pain. However, many prescribers are reluctant to prescribe methadone due to its interactions with other medications and effects on heart rhythm. Methadone also has a long and variable half-life which requires patience during dosing. Our presentation seeks to provide information to APNs on how to use methadone effectively and confidently. We will review indications for methadone and review safe prescribing practices. We will review some of the most common problems we see in prescribing methadone and walk through our strategies. We will also use case studies to provide real world examples of successful use of the medication.

Speakers
Mary Beth Sheehy is a nurse practitioner practicing in palliative care at Advocate Christ Medical Center and in critical care at Northwestern Memorial Hospital. Mary Beth graduated with her BSN from Marquette University in 2009 and her DNP from Rush University in 2017. Mary Beth worked in critical care and post-operative care before transitioning to palliative care in 2022. Mary Beth enjoys being part of the government relations committee on ISAPN and lecturing on various topics in the Acute Care and Gerontology program at Rush University.
Katija McCarthy is a nurse practitioner practicing at Advocate Christ Medical Center. She received her Masters of Science of Nursing from Resurrection University in Chicago in 2015. She has been practicing nursing in various specialties for 22 years since graduating from the Chicago Public Schools Practical Nursing Program in 2002. She has taken the scenic route in nursing, beginning her career as an LPN, and arriving at APN with Full Practice Authority. She has been practicing as an Advanced Practice Nurse in the Palliative Care department at Advocate Christ Medical Center since 2018 (inpatient and outpatient clinic) and is ACHPN certified. Her motto which drives her practice in the field of nursing is 'always working on mastering my craft.
